Your nutrition label will only be accurate if you provide us with all of the information requested below. We must have a list of all product ingredients and the weight of each.

In the first blank please provide the exact name for each ingredient in your formula (recipe) Be as specific as possible.
If you use any brand name products, provide the brand of that product in the second box. For example, if you use commercial crushed tomatoes, you would put "crushed tomatoes" in the first box and "Hunt's" in the second box. If no brand name products are used, leave this box blank.
In the third box identify the amount of the ingredient used and in the last box, the select the units of measure for this amount from the list provided. Please note: for accuracy, amounts must be provided as weights (grams, wt. ounces, etc.) rather than as volumes (teaspoons, tablespoons, cups, etc.)
